Print the required photos on photo paper (or get them printed at Walmart, London Drugs, etc.)
These photos MUST be printed on photo paper
These photos MUST meet the minimum size requirement (5cm x 3.5cm)
These photos MUST be a clear shot of your face and shoulders, if you need to, ask someone to take the photos for you and crop it to include your full head/hair and your shoulders

There is also a Child Safety Waiver form and a Consent for Private Family Visits (PFVs) form should you need them, also, submit the Common Law form if necessary for PFVs
Double-check and triple-check that all areas are filled out. If something is missing, they will mail back/return your ENTIRE application, delaying the process.
You will have to renew your visitation permissions/reapply every 2 years. Your loved one will be notified when your visitation expires, but it’s recommended to set a calendar reminder or take note of the date when you’re first approved.
You must mail your forms in according to the instructions outlined on the website. We recommend sending all forms (visitation or otherwise) via TRACKED MAIL.
Your loved one will receive confirmation that you have been added to their visitation list.
